Instructions for Ordering Products
Unfortunately we are not equipped to process a credit card, however there are two
ways of ordering product from Healthy Bodies.
1.) Print
out order form below and complete your order. To print the form, left click at the top
of the form and hold. Drag the mouse down and highlight the form in blue. Go to file in you task bar and
click print. Click on selection, then O.K. Fax or mail your fcompleted form to

2.) Call
530-832-4757 to speak to a representative from Healthy Bodies, leave your order on voice
mail along with a return telephone number.
3.) Send your check or money order to the address above.
